Appeals
Title Description
Appeal Form If Council or authorized staff do not approve a person's Building Permit/Development Application, such person can appeal the decision to the St. John's Local Board of Appeal. However, an appeal can only be initiated if the rejection of the application is made under the St. John's Development Regulations.  
Arts and Culture
Title Description
Art Procurement Submission This form must be completed by artists and commercial galleries submitting to the City of St. John's Art Procurement program.
Final Report for Grants to Artists (Individual) This form is required to be completed and submitted by any individual artists who have received a City of St. John's Arts Grant. The final report must be submitted by the following year’s grants deadline.
Final Report for Grants to Arts Organizations This form is required to be completed and submitted by any arts organization that has received a City of St. John's Arts Grant.  The final report must be submitted by the following year’s grants deadline.
Poet Laureate Application This form must be completed by those wishing to be considered for the position of Poet Laureate for the City of St. John's.  Before completing the form, applicants should read the Terms of Reference and Selection Process for this honourary position.

2024 Service Station Questionnaire Pursuant to Section 8 of the Assessment Act 2006, it is requested that income and expense information pertaining to the operation of the applicable property be forwarded to the Assessment Division. Business Operators Information This form is required to be completed by all new business owners.  
Commercial Property Tax Allowance Claim - FIN-1004 This form is required to be completed by commercial property owners wishing to submit a commercial property tax allowance claim.
Leasable Area Questionnaire - FIN1002 This form is required to be completed by commercial property owners paying the Business Realty Tax to the City of St. John's.
Pre-Authorized Payments for Commercial/Mixed Use Properties This form is to be completed by those who wish to have personally approved debits for municipal taxes applying to Commercial/Mixed Use Properties.
Pre-Authorized Payments for Residential Properties This form is to be completed by those who wish to have personally approved debits for municipal taxes applying to residential properties.
Quarterly Accommodation Tax Report The Accommodation Tax (as known as the Tourism Marketing Levy) provides a sustainable source of funding for the marketing and promotion of tourism to the City through our partner Destination St. John’s (DSJ).  The Accommodation Tax is set at 4% which is to be collected from any person who for a daily charge, fee or remuneration is accommodated, lodges or stays in a room in a building within the City that is licensed under the Tourist Establishments Act or owned by Memorial University A Quarterly Accommodation Tax report is required to be remitted showing the total room revenue and the amount of the levy being remitted.
Request for Tax Certificate Information This form is required to be completed by law firms seeking the tax status of property situated within the City of St. John's.
Residential Condominium Unit Questionnaire Pursuant to Section 8 of the Assessment Act 2006, it is requested that the completed Residential Condominium Unit Questionnaire, for owners in receipt of notification by the City, be forwarded to the Assessment Division by April 30, 2022.
Seniors Tax Reduction Form This form is to be completed by senior citizens who are eligible for a reduction in City taxes.  
Water Tax Exemption for Subsidiary Apartments This form is required to be completed by a residential property owner seeking a water tax exemption for the subsidiary apartment that is not being rented or occupied as an apartment. 
Awards
Title Description
Building Healthy Communities Award Application This form must be completed by those wishing to nominate an individual and/or group who have volunteered their time to enhancing the community by providing the opportunity to live healthier lives within the City of St. John's.
Heritage Award Program The Heritage Awards recognize commercial and residential property owners for their work in rehabilitating, restoring and celebrating our rich built heritage.  
Senior of the Year Nomination Form The City is seeking nominations for the Senior of the Year Award. The City of St. John’s will recognize a senior citizen who has made significant volunteer contributions to the city through improving the quality of life of citizens through community involvement (music, sports, recreation, arts, etc.) and/or service to the community.
Tourism Awards This form is required to be completed by any person and/or group wishing to nominate a person and/or group for one of the annual tourism awards presented by the City of St. John's.  Refer to the awards criteria, at the end of the nomination form, for a detailed description of each award before completing and submitting the nomination form.  
Youth of the Year The Youth of the Year awards are designed to honour a junior high and a high school student in St. John's, who through their volunteer contributions, extracurricular activities and community involvement have helped to make St. John's a better place to live.  This application must be completed and submitted to apply for, or nominate a candidate.

St. John's Local Immigration Partnership (LIP) Expression of Interest Form To express interest to volunteer with the St. John's Local Immigration Partnership (LIP), this form must be completed and submitted as outlined in this document. Vacancies will be filled on the main leadership body, Partnership Council and topic-specific Working Groups.
Volunteer Application Package To apply to volunteer with the City of St. John's this application form must be completed and submitted as outlined in this document.

Development Applications and Building Permits
Title Description
Application for Building Permit For Building permits. Required documentation must be attached with application, and signatures are required.
Application to Connect This application is to be completed by those property owner's looking for a permit to connect to the City water and/or sanitary/storm sewer services.  
Construction of Seasonal Parklet For applicants who wish to locate a Parklet (Outdoor Eating Area and/or Lounge) on City land, in the Downtown or Churchill Square area and are within a Commercial Zone where these Uses can be considered.
Electrical Permit Application The permits for all electrical work can only be issued to electrical contractors licensed by the City of St. John's.
Heritage Building Designation Application The City of St. John's Heritage Designation Program recognizes homes, buildings and structures which are significant due to their architecture, history, environment and/or their integrity. 
Heritage Financial Incentives Program The Application Form for the Heritage Incentive Program must be completed and submitted to the City of St. John’s between March 1 and May 1, annually. Type of Grants: Heritage Maintenance Grant Heritage Conservation Grant Permit Waiver Applications must include: a completed application form; current, coloured photographs of the façade directly abutting a public street, with close-ups of the areas of work. Photographs may be submitted digitally by email or USB drive; two cost estimate quotes for the proposed work; and, supporting documentation substantiating the amount of increased costs attributed to the designation as a municipal Heritage Building or due to its location in one of the City’s Heritage Areas. Applications will not be considered complete unless all required, supporting documentation has been submitted. Late or incomplete applications will not be reviewed.
Municipal Compliance Information Request The request for municipal compliance information is typically submitted by law firms on behalf of property owners or perspective buyers of a property. The form would include information on zoning, the number of units, outstanding permits or work authorizations attached to a specific property.    
Parklet Checklist - City Owned Land For applicants who wish to locate a Parklet (Outdoor Eating Area and/or Lounge) on City land and are within a Commercial Zone where these Uses can be considered in either the Downtown or Churchill Square, the following checklist outlines the application process.

PER 3204 - Assurance of Field & Compliance Form This form is required to be completed by a professional engineer after the completion of a project and before the issuance of an occupancy permit verifying that the work complies with the plans and supporting documents submiitted in support of the application for the building permit.  
PER 3205 - Commitment for Field Review Form The form is required to be completed by a professional engineer/architect confirming responsibility for field review of specific components of construction prior to the issuance of a Building Permit by the City of St. John's.  
PER 3206 - Fire Safety Evaluation Form This form is to be completed when applying to develop or renovate an upper storey property using the standards set out in the report Fire Safety Alternatives for Upper Storeys. For application guidelines and information on how to apply, go to the page upper storey development. A completed PER 3003 - Building/Development Application must also accompany this questionnaire.
PER 3212 - Residential Dwelling Water Pipe Sizing Form This form is required to be completed by the plumbing contractor.
PER 3223 - Mobile Vending Permit Application Application to sell in mobile locations.
Planning/Development Application Planning/Development Application is the general application used by the Department of Planning, Engineering and Regulatory Services. Planning/Development Application is the general application used by the Department of Planning, Engineering and Regulatory Services. This form should be completed, along with the appropriate corresponding development checklist, when applying for development applications such as, but not limited to: rezoning, discretionary use application, home occupation, home office, development, subdivision development, etc. Development approval must be obtained prior to the start of any of these activities. It is the owners’ responsibility for compliance of any conditions attached to an approval.  
Plumbing Permit The permits for all plumbing work can only be issued to Plumbing Contractors licensed by the City of St. John’s.

Inclusion, Update, or Removal on Voters List Use this form to be added or removed from the voters list or to have your information updated. This form must include a copy or copies of acceptable identification.
Nomination Form Use this form to be nominated for a City of St. John’s Municipal Election.
Nomination Form - Not Present Use this form to be nominated for a City of St. John’s Municipal Election if the candidate is unable to attend the nomination appointment.

Oath or Affirmation of Fraud Use the form if you believe your ballot has been used by someone else to vote.
Oath or Affirmation of Secrecy Use this form to affirm that any and all information received from the Voters List will be used solely for the purpose of the municipal election and will not otherwise be used or shared.
Oath or Affirmation of Voter Use this form to verify you have not previously voted in the current election and that you are eligible to vote.

Grants
Title Description
Capital Grant Non-profit groups and organizations seeking financial assistance for the construction, expansion or renovation of a facility that will meet a demonstrated need in the areas of recreational, cultural or other community service may apply for a Capital Grant. Limited funds are available to assist non-profit groups or organizations within the City to construct or renovate community facilities where the applicant can demonstrate the need for the proposed facility as well as the ability to maintain it and fund the programs to be provided in the facility.

Grants to Artists Individual The City of St. John's makes financial resources available to eligible individual artists in the following disciplines: Theatre Dance Music Literary Arts Film and New Media Visual Art and Craft Please see the Community Services Grant Policy and the Community Services Grants Procedures.

Housing Catalyst Fund Application The Housing Catalyst Fund supports practical and collaborative projects which lead to larger change in the sector. The City’s role through this fund is to work with community groups and other stakeholders (housing providers, service providers, non- profit organizations, builders/developers) to facilitate and plan housing solutions that will enhance the quality for life for individuals and families and build a healthier community. To be considered, projects must be solution focused and action oriented and fill a clearly identified need in the housing sector. Catalyst projects are intended to be concluded in approximately 1 year. Emphasis will be placed on projects that are led by or include a community agency. Applicants must provide an incorporation number. Please note this funding is only available to groups / projects based in St. John's. Applicants can apply for up to a $30,000 grant per project. The application deadline is Friday, April 15, 2022. The Catalyst Fund is a partnership between the Community Housing Transformation Centre and the City of St. John’s. *Please see the Application Package which outlines the funding restrictions. Projects that align with the City’s priorities but do not align with the Centre’s priorities are only eligible for up to a $10,000 grant.
Meeting, Convention, and Sporting Event Grant The City of St. John's supports non-profit groups and organizations seeking financial assistance to host meetings, conventions, and sporting events within the city. The meeting, convention, or sporting event shall have at least 75 percent of delegates, athletes, coaches, and/or officials from outside the province, in order to be considered eligible.
Special Events and Festivals Grant Application Not-for-profit organizations producing multi-faceted programs of one or more days, taking place within the City of St. John’s, may apply for a Special Events & Festivals Grant. Each event should offer participants a unique experience and appeal to a significant potential audience.
Sport Grant Application The City of St. John’s recognizes the importance of sport in the community. Our goal is to assist not-for-profit youth organizations with resources that will enhance participation and maximize use of facilities.
Youth Travel Grant Application Youth is defined as up to 25 years of age when registered and attending secondary or a post-secondary institution. Youth Sport Travel Travel funding assistance is available to youth residents of St. John’s who have won the right to participate in amateur championship sporting events. Funding for such travel is restricted to atlantic, national or international events. Youth Non-Sport Travel Travel funding assistance is available to youth residents of St. John’s who are travelling out of province to attend formally sponsored symposiums, conferences, seminars, competitions, etc., dealing with a wide range of interests including educational, cultural, social, political, which are designed and deemed to broaden ones horizons in these disciplines.

Animal Licensing Application Form - PW1005 This application is for the registration of pets within the City of St. John's. Residents are urged to have their pets licensed as the benefits are numerous. By having a direct link to the owner's contact information, the City is quickly able to idenify the owner of a missing and/or injured pet.
Cat Adoption Questionnaire - PW-1011 This questionnaire is required to be completed by any person seeking to adopt a cat from  the City of St. John's Humane Services facility.
Cat Neuter Assistance - Male Only This form is to be completed by residents of St. John's seeking financial assistance to neuter their male cat.  Male cats must be 6 months of age of older to qualify for this program. The completed application form and the following information are required for consideration for this assistance program: Current year income tax assessment for all family members living in the same household  GIS letter (if you are a senior) Proof of St. John’s residency Payment
Cat Spay Assistance – Female Only This form is to be completed by residents of St. John's seeking financial assistance to spay their female cat.    Female cats must be 6 months of age of older to qualify for this program. The completed application form and the following information are required for consideration for this assistance program: Current year income tax assessment for all family members living in the same household  GIS letter (if you are a senior) Proof of St. John’s residency photo identification
Dog Adoption Questionnaire - PW-1012 This questionnaire is required to be completed by any person seeking to adopt a dog from the City of St. John's Humane Services facility.
Pet Safekeeping Foster Application Humane Services is committed to working with the RNC and women’s shelters to provide temporary accommodations for animals whose owners are fleeing from intimate partner violence. Victims and their children delay leaving their households out of concern for the family pets. This increases risks to the victims, children and the pets of further injury or death. Emergency shelters do not accommodate pets. You can help by fostering a pet in your home. All you need to provide is a safe and loving environment for up to 30 days. Complete this form and email it to humaneservices@stjohns.ca.  
Waiver of Euthanasia/Surrender Fee Application This form is required to be completed by any person seeking assistance from the City to have the fee waived for the euthanisation of their pet.

Claim Form General - LEG1000 This form is required to be completed by any person making a claim against the City for property damage and/or bodily injury.  
Land Sale Request This form is required to apply to purchase or lease City property.
Municipal Diversion Application Form Municipal Diversion is a voluntary compliance program that enables a person charged with a municipal offence the opportunity to avoid prosecution by completing certain requirements.  
Snow Clearing Damage This form is required to be completed by any person making a claim against the City for damage to property caused by snow clearing operations.  
Tolerance Request This form is required to apply for a tolerance for existing encroachments on City property.
Licensing

PER 3215 - Electrician Journeyman's License Application This application is required to be completed in order to obtain an Electrician Journeyman's License from the City of St. John's.  
PER 3216 - Electrical Contractor's License Application This application is required to be completed in order to obtain an Electrical Contractor's License from the City of St. John's.  
PER 3217 - Plumber Journeyman's License Application This application is required to be completed in order to obtain a Plumber Journeyman's License from the City of St. John's.  
PER 3221 - Mobile Sign Permit Application This form is to be completed to obtain the permit required for each mobile sign placed or erected on a site within the city of St. John's.
PER 3222 - Mobile Sign Annual Contractor's License Application This application is to be completed annually by mobile sign contractors to obtain a license under the Mobile Sign By-Law which permits them to place, erect or lease mobile signs within the city of St. John’s.  
PER 3223 - Mobile Vending Permit Application This application is to be completed to obtain a license to conduct sales from a table, stand, cart, or vehicle outdoors, on a public street or private commercial property.
PER 3224 - Amusement Machine Establishment License Application This form is required to be completed by establishments operating video lottery and amusement machines within the city in order to obtain a license. There is a license fee per machine annually.  
PER 3225 - Amusement Machine Operator's License Application This form is required to be completed annually by companies who own and lease amusement machines within the City of St. John's in order to obtain the proper license.  
PER 3226 - Churchill Square Open Air Market Application This form is to be completed in order to apply for one of the 14 parking stalls available at the Churchill Square parking lot for outdoor vendors, generally referred to as the 'open air market'.
PER 3227 - Plumbing Contractor's License Application This application is required to be completed in order to obtain a license for a Journeyman Plumber with Inter-Provincial Qualifications who possesses at least two years experience as a Journeyman Plumber, or to a company who employs such a person to manage its plumbing contracting department.

Access to Information Request The Access to Information Request form is a means to obtain information that is not routinely disclosed by the City of St. John’s. Contact the ATIPP Coordinator for more information.
Copyright Materials - One Time User Agreement This is a one time copyright user agreement to publish, exhibit or broadcast archival photographic materials.
Correction of Personal Information The Access to Information and Protection of Privacy Act, 2015 gives people the right to request a correction of their personal information. If you believe that information about you contained in a City of St. John's record is inaccurate, please contact the office having custody and control of the record and request to have it corrected. If that approach is unsuccessful, you may file a formal request by completing a Correction of Personal Information Request form.
Delegation Request Form - COTW This form is required to be completed by any group/organization/business or individual wishing to present to the Committee of the Whole (COTW).

Letter of Congratulations from the Mayor - Request This form is required to be completed if you would like a Letter of Congratulations from the Mayor for events that meet the criteria noted.
Letter of Support from the Mayor - Request This form is required to be completed by anyone wishing to obtain a Letter of Support from the Mayor to include with applications for funding to host an event within criteria noted.
Marriage Service Request Please complete the attached request form for Marriage Services at City Hall.
Message from the Mayor - Request This form is required to be completed if you would like a Message from the Mayor for arts and cultural events, conventions, and trade shows held within the City of St. John's.
Privacy Complaint If you believe that your personal information has been breached in any way, please complete this Privacy Complaint form and submit it to the City’s ATIPP Coordinator. The ATIPP Coordinator will contact you regarding your complaint and work towards a resolution.
Proof of Authority The Proof of Authority Form is used to designate an authorized representative to make a Personal Information Request on your behalf. This form must be attached to an Access to Information Request Form in order to fulfill the request.
Protocol Event Request This form is required to be completed for Proclamations (please see attached Proclamation template, to be completed as well), Flag/Banner Raising and Lighting requests. Requests must be submitted at least two weeks prior to the date of the event.
Request for Mayor to Speak/Attend your Event This form is required to be completed if you would like the Mayor to attend an event that your organization is hosting.  All requsts must be submitted at least three weeks prior to the event.
Room Usage Agreement This form is required to be completed by any person or group looking to book the Foran/Greene Room or the Wyatt (Great) Hall at City Hall.
Parking Services
Title Description
Accessible Parking Space Permit This form is required to be completed by any person wishing to obtain an Accessible Parking Space Permit for designated parking areas within the City of St. John's.
Disclosure Form for Parking Tickets Disclosure is all relevant evidence (photos, officer notes, etc.) related to your ticket.
Parking Application This appliation is required to be completed by any person seeking a parking permit for use in designated areas within the City of St. John's.
Service Provider Parking Permit Application Form - ENG-1000 This application is required to be completed by any business seeking to obtain parking permits for staff  providing a service to residential properties.  
Tour Operator Parking Application The Cruise Ship Tour Operation Application and Parking Reservation forms are to be filled out by operators who wish to park on Harbour Drive and pick up passengers from cruises. Once submitted the City will review the application, advise if this is approved, and confirm the parking space reservation amount to be paid. Only those on the pre-approved list for each cruise will be permitted to park on Harbour Drive.
Public Works and Parks
Title Description
Commemorative Program Application Form The Department of Public Works offers commemorative programs to memorialize the loss of a loved one, or to commemorate a birth, anniversary, graduation or special occasion. To apply for the program, please complete and submit the attached form.
Memory Stone The Bannerman Park Foundation accepts applications for Commemorative Stones to be placed in Bannerman Park. Stones may be purchased to memorialize the loss of a loved one, or to commemorate a special occasion.   
Residential Re-Leaf Residential Re-Leaf is an annual program through which homeowners within the City of St. John’s can apply for a $200 voucher towards the purchase of a tree to plant on their property.  Applications for 2022 will be accepted until 4 p.m. on Friday June 24, 2022. Adaptive Equipment Lending Program Hiking, skiing, sliding and skating should be enjoyed by everyone and we have the equipment to help. Borrowing adaptive equipment removes financial and ability related barriers.
Adult Program Subsidy Application Financial support is available for individuals facing financial barriers to participate in City of St. John's Recreation Division activities.

Application for Refund, Credit, Transfer or Family Discount This form is required to be completed by persons seeking a refund, credit, transfer or family discount for a recreation program with the City of St. John's.
Attendant Pass Application This application is required to be completed by any person with a disability who wishes to participate in a recreation activity of their choice with the support of an attendant. Attendants will be permitted to attend recreation activities and events free of charge when providing support for a person living with a disability. Children's Program Financial Support The City of St. John's recognizes the need for support services to participate in programs of one’s choice. Subsidy programs provide opportunities for families facing financial barriers to participate in recreational programs offered by the City of St. John’s.
Community Garden Registration This form is to be completed by those wishing to book a plot at the Paul Reynolds Community Garden. Please review the garden Code of Conduct within the form before submitting the comleted form.
Consent to Leave This form is to be completed by parents/guardians who give consent for a minor to leave a registered program.
Facility Rental This application must be completed by those wishing to rent the Paul Reynolds or H.G.R. Mews Community Centres.
Inclusion Support Application This application is required to be completed by persons seeking inclusive services for participants enrolled in or enrolling in programs through the Recreation division in the City of St. John's.
Medical Condition Emergency Plan This form is to be completed if the participant has a medical condition (i.e. diabetes, heart condition, etc.) other than allergies or seizures which may impact ability to participate in a Recreation Division program. Please note, if medication is required to be administered, a Medication Consent form is also required.
Medication Consent This form is to be completed by the guardian of a participant under 19 years if medication is required to be administered at the program site by Recreation Division staff. Medication is only administered during full-day programs.
Outdoor Sports Facility Rain-Out Complete this form after the outdoor recreation facility a league/organization rented was rained out. Familiarize yourself with the rules, regulations, and policies for rental through the Outdoor Sport Facility Package prior to completing the rain-out form.

Volunteer Junior Recreation Leader This form is to be completed and submitted by youth ages 14 years or older and currently registered in junior high or high school wishing to volunteer with a City of St. John’s Recreation program. Youth Art Exhibit Submission This form is required to be completed by any youth ages 12 to 18 who would like to submit a piece of orginal artwork to the City of St. John's annual Youth Week art exhibit.
Youth Talent Show Performer Application This application is required to be completed by any person and/or group between the ages of 10 to 16 years-old who would like to enter the annual Youth Talent Show presented by the Department of Community Services. 
Waste Management
Title Description
Garbage Cart Mobility Exemption This form is required to be completed by those requesting an exemption from using a cart for curbside garbage collection. Applicants must also provide medical verification from a healthcare provider or relevant professional to support they would have difficulty maneuvering a cart to a regular collection point. 
Landfill Vehicle Operating License This application is required to be completed by any person wishing to obtain a landfill permit for vehicles carrying garbage and recycling to the Robin Hood Bay Waste Management Facility.
