Medical Condition Emergency Plan

This form is to be completed if the participant has a medical condition (i.e. diabetes, heart condition, etc.) other than allergies or seizures which may impact ability to participate in a Recreation Division program. Please note, if medication is required to be administered, a Medication Consent form is also required.
